($7 million lifetime max on CeltiCare Plans, $2 million on Short-Term)

Celtic DOES have a 12-month rate guarantee. 

All CeltiCare Plans include $500 Supplemental Accident at no additional cost.
Celtic does not offer a maternity benefit.

The CeltiCare Plus Option (not available with the Short-Term or Basic Plans).  Includes the Healthy Lifestyle Program, Rx Drug Card, and Preventive Care.  Please note, however, that the Celtic plans WITHOUT the Plus Option only come with an Rx Discount Card.

CeltiCare Select PPO 80/20 Plan: 80/20 coinsurance to $10,000, $15 copay for in-network physician visits (limit 6 per person per year, 7th & subsequent visits subject to deductible and coinsurance).

$500 deductible - $1000 deductible - $1500 deductible - $2500 deductible - $5000 deductible

CeltiCare Select PPO 100% Plan: No Coinsurance, rest of coverage is same as Select PPO 80/20 Plan.

$2500 deductible - $5000 deductible

***Important: Celtic does not offer a monthly pay option for their Short-Term Plan.  The Celtic Short-Term Plan is something to look at if you have a good idea of how long you will need short-term coverage.  If you do not know how long you will need short-term coverage, then one of the other Short-Term Plans may be what you should look at.  Approval takes about 2-3 days.  Effective on January 1, 2003, Celtic Short-Term became a one-term policy with a choice of one to six months of coverage.  There is  no longer an option to reapply for another term. **

Short-Term: Once deductible is satisfied, 80/20 coinsurance to $5,000, then Short-Term pays 100% of covered expenses.  Deductible applies to doctor visits. May have an effective date as soon as the day after you send the application.

Multiply the monthly premium shown below by how many months you will need coverage will equal what your short-term premium will be.
